As a Health Access Navigator, you help patients make sense of complex health insurance and coverage systems so they can get the care they need. You listen to questions and concerns, explain plan options in clear language, assist with applications and renewals, and problem-solve issues related to costs, billing, eligibility, and benefits. You also support patients with related barriers such as transportation, digital access, or paperwork literacy. Your work makes a direct difference in a patient’s ability to access medical, dental, behavioral health, and specialty care. If you enjoy learning systems, translating information, and helping people feel confident and supported, this role matters. Patients rely on you to reduce confusion, remove barriers, and ensure that care is possible-financially, logistically, and practically.

For 65 years, HCHC’s mission has been to provide accessible, affordable services to ensure the health of all Hilltown residents. We’re dedicated to the health and wellness of our Hilltown communities and provides a wide range of services at our four sites. In addition to medical and oral health/dental services, we provide behavioral/mental health services, substance abuse treatment, eye care, physical therapy, and laboratory services. We also provide numerous community services and supports. Federal funding supports some of this work, as do insurance payments, but these are not sufficient to cover all the services we provide. We rely on grants from foundations and other sources, as well as on individual donations, to support new initiatives and all of our community programs.
