Bayside Health is a public health service delivering high-quality care across every stage of life for close to 1.2 million people living in metropolitan Melbourne, the Mornington Peninsula, Koo Wee Rup, Bass Coast and Southern Gippsland. The Department
HARP is a service within the range of Health Independence Programs (HIP), and reports to the Acute and Integrated Care clinical stream of the Home Acute and Community (HAC) program. HARP aims to prevent avoidable hospital presentations and admissions. The Alfred Care Group HARP service achieves this through multidisciplinary and interdisciplinary models of care, providing services to people with chronic needs and or complex psychosocial backgrounds.
The Role
The HARP complex care Case Manager is responsible for providing short-term case management and care co-ordination to clients who have complex psychosocial issues and chronic health conditions that place them at risk of potentially avoidable hospital presentations. Case Managers work in a multidisciplinary team utilising an interdisciplinary approach to care. On a day to day basis, the role is overseen by site based HIP Complex Care Coordinators(HIP CCC). Direct Care, including comprehensive social work or equivalant assessment and clinical intervention, is delivered within the context of an individual care team for each client. Clinical interventions that require medical management are overseen by the HARP registrar. The Case Manager holds a caseload of clients and works directly with clients to develop client-centred SMART goals. In collaboration with the multidisciplinary team, the case manager develops a holistic approach to achieving these goals set out in the client’s care plan. Clinical interventions and case management strategies are individualised and flexible to ensure effective and efficient service delivery.
