The Department

The Caulfield Community Health Service (CCHS) is co-located on the Caulfield Hospital site, and provides a range of clinical and health promotion services for people of all ages who live, work or study within the local community. These multi-disciplinary services are provided in the centre, community, home or via Telehealth. Following assessment, the work will largely involve equipment prescription, home modifications, and referral and liaison with other services to increase or maintain independence, and reduce risk in the home. The majority of the clients will be over 65 and referred via My Aged Care under the Commonwealth Home Support Program.

The Role

The Occupational Therapy position will sit within the Adult Health team and you will:

  • Provide comprehensive assessments, recommendations and OT services to clients living at home
  • Develop and implement person centred care plans
  • Work within a multi-disciplinary team to ensure a holistic approach to client support
  • Liaise with relevant agencies to coordinate, facilitate and communicate client treatment and outcomes. This could include local councils and contractors to facilitate installation of home modifications, and equipment suppliers
  • Demonstrate an understanding of pathways within the health care system in order to help facilitate appropriate care and advocate for clients with complex care needs (e.g. My Aged Care, Commonwealth Home Support Program, Home Care Packages)

Qualifications and Experience Required

  • Degree in Occupational Therapy
  • Registration with the Australian Health Practitioner Regulation Agency (AHPRA) and adherence to continuing professional development requirements
  • Demonstrated occupational therapy clinical experience relevant to the community health sector (mimimum 2 years experience)
  • Current Victorian driver's licence
