About the role

We have an exciting opportunity to join Haringey Alcohol and Recovery Service (HAGA) as a GP Hub Recovery Coordinator.

As a GP Hub Recovery Coordinator, you will work as part of a dedicated multi-disciplinary team, supporting people affected by substance use who are accessing support through GP and primary care settings. You will play an important role in engaging people early, helping them to understand the impact of their substance use and supporting them to access the right treatment, recovery and community services.

You will work closely with GP practices, healthcare professionals, service users and partner agencies to provide a joined-up approach to support. The role will involve working with people on a one-to-one basis, as well as contributing to and facilitating peer support groups and other community-based interventions.

The role will suit someone who is positive, motivated and able to work confidently with people who may have a range of complex needs. You will be comfortable working in a fast-paced environment and developing effective relationships with professionals across primary care and the wider community.

Direct experience of working in substance use services is desirable but not essential. We welcome applications from people with transferable skills and experience who have a genuine passion for supporting people to achieve positive change.

Key responsibilities

• Manage a caseload of service users, devising, implementing and reviewing personalised recovery and treatment plans, working closely with colleagues, family members and significant others where appropriate.
• Carry out triage, screening and comprehensive assessments, identifying individuals' needs, strengths and risks and ensuring they are supported to access appropriate treatment and recovery pathways.
• Work collaboratively with GP practices, healthcare professionals and partner agencies to provide joined-up support and promote progression from initial engagement through structured treatment towards sustained recovery.
• Facilitate access to treatment, peer support and community resources, helping service users to build their recovery and social capital and supporting them to develop sustainable support networks within their local community.
• Develop and deliver high-quality psychosocial interventions, group work and one-to-one support, helping individuals to build confidence, self-efficacy and the skills needed to achieve their recovery goals.

About you

• You will have experience of working in a client-facing role, with the ability to build positive and trusting relationships with people from a range of backgrounds and circumstances.
• You will be compassionate, non-judgemental and person-centred, with a genuine belief in people's ability to make positive changes and take control of their recovery journey.
• You will be confident working collaboratively with professionals and partner agencies, including colleagues within primary care, and able to communicate effectively across a multi-disciplinary team.
• You will be organised and adaptable, with the ability to manage a caseload, prioritise competing demands and work effectively in a fast-paced and sometimes challenging environment.
• You will be motivated to develop your knowledge and skills, with an understanding of or interest in substance use, health inequalities and recovery. Direct substance use experience is desirable, but transferable experience will also be considered.
