Duties and Responsibilities
This position supports Adult Protective Investigators and Human Services Counselors with coordinating placement services for Vulnerable Adults. This position requires a highly independent worker, responsible for the coordination of every aspect related to placement, to include staffing cases with investigators to find the least restrictive placement alternative for APS clients. The Placement Coordinator will join efforts with other State agencies and serve as the services liaison with the Department of Elder Affairs, Agency for Health Care Administration, and Agency for Persons with Disabilities, with the goal of preventing future abuse, neglect of exploitation of the Vulnerable adult requesting placement services from the Adult Protective Services Program.
Knowledge, skills, and abilities, including utilization of equipment, required for the position: Knowledge of the Medicaid Long Term Care Program, and how to successfully submit service referrals for APS clients. In addition, responsible for coordinating facility closures with the Agency for Health Care Administration and assisting AHCA with placement services as requested. Knowledge of the principles and techniques of effective communication.

This is an independent and complex professional consultative position in the Adult Protective Services (APS) Program Office. Provides statistical support for APS programs regarding placement services offered by the program to Victims of abuse, neglect, and exploitation, to include development of data collection instruments, data collections, data analysis, and preparation of comprehensive reports. The Placement Coordinator will develop and recommend planning strategies to enhance and increase field performance and quality of services provided to the community.
Responsible for data analysis, tracking and reporting outcomes for services offered to APS clients. Analyzes designs, documents, and implements internal reporting systems and procedures for services, some of which are used to evaluate operational effectiveness and efficiency.
Works in tandem with the APS Central Office to provide a "check and balance" to ensure all reported information is accurate for monthly Performance Statistics. Track performance on established standards by unit and publish results region-wide as needed. Makes information available to region management, Headquarters Program Office and collect and analyze data to determine trends, organizational and technological projections, comparisons, cause effect relationships, and correlations intended to support performance reviews, help determine root causes, and help set priorities for resource use. Use performance measurement data for fact-based decision making, setting, and aligning organizational directions.
Acts as the Region's TES coordinator, covering all facets of the TES program from keeping track of potential clients, reviewing invoices and paperwork submitted for accurateness, follow-up for timely submissions, confirming successful payments by ASSC and minimizing budget surplus/deficit. This includes monthly projections of TES budget surplus/deficit for the fiscal year.
Responsible for ensuring high quality and sufficient Adult Protective Services and relate professional training for community and service providers to include law enforcement, the State Attorneys' Office's, and various health care facilities, including Assisted Living Facilities and Nursing Homes. Develops and delivers community education programs to increase public awareness of adult abuse, neglect, and exploitation as well as available preventative services. The position is also responsible for coordinating outreach activities to educate community service providers, law enforcement, and others about the agency's services, to include placement services.
