Job Location: Goldthwaite, TX 76844JOB TITLE: Licensed Clinical Social Worker
DEPARTMENT: Rural Health Clinic REPORTS TO: Clinic Manager
JOB SUMMARY: Responsible for day-to-day clinical operations of the Department’s Behavioral Health Counseling Services. This individual will also take part in the RHC provider collaboration to provide needed services to our population. OSHA Bloodborne Pathogens Class I.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
A. Provide individual therapy/ counseling services as scheduled.
B. Assure all tasks performed are within span of control and scope of clinical license and in compliance with related State and Federal guidelines, rules and regulations.
C. Ensure patient dignity and respect patient values. Operate within ethical standards.
D. Assess and reassess with the treatment team the patient’s treatment needs.
E. Develop, monitor, and evaluate the patient’s treatment plans and progress.
F. Participate in the referral development process.
G. Communicate effectively with families and agencies, conveying a positive, professional image. Provide resources and support for patients before, during, and
after treatment.
H. Ensure timely and accurate documentation of all assessments and progress notes.
I. Communicate results and trends to leadership on ongoing basis regarding concerns,
improvements, changes, etc. Identify areas needing improvement and utilize the facility performance improvement process as directed.
J. Coordinates Department’s patient clinical outcome measurement process.
K. Participates in Department-wide meetings to maintain two-way communication, problem solving, and information passing a minimum of once a month.

A. Maintains confidentiality while working with confidential matters on a daily basis.
B. Adheres to Hamilton Healthcare System’s Behavioral Standards. C. Emulates the customer service expectations for Accountability, Respect, Integrity, Service, and Excellence. D. Maintains HIPAA compliance at all times.
E. Abides by the Hamilton Healthcare System Personnel Policy and Employee Handbook.
F. Performs all other duties as assigned.

QualificationsJOB QUALIFICATIONS:
A. Education: Clinical Social Work
B. Personal job-related skills: Must have good understanding of state and federal regulations that govern the practice of Social Work. Expert understanding of the clinical group therapy process and related clinical dynamics. Excellent clinical documentation skills. Expert communication and clinical skills. Must have the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, clinical documents or governmental regulations. Expert decision making skills and critical thinking ability; must perform daily duties independently with little support.
Must be computer literate and possess working knowledge of fax, copier, and scanner.
C. Licensure, registry or certification: Current LCSW License in the State of Texas

Experience:
a. Prior Work

Experience: Experience in initial clinical assessment and admission work, discharge planning and patient chart development preferred. Experience in Trauma Informed Care Methodology is preferred. Training or Experience in non-violent crisis intervention is preferred.
b. Technical Training: Current American Heart Association CPR training.
E. Physical and Mental Requirements: Regularly required to use hands to finger, handle, or feel objects, tools, or controls; talk and hear. Required to sit and reach with hands and arms. Required to stand, walk, kneel, and crouch. Must regularly lift and/or move up to 10 pounds and occasionally lift and/or move up to 40 pounds
