Gilead Community Services is seeking an attentive and recovery-oriented Social Rehabilitation Counselor to join our multi-disciplinary program staff.
About the position:
The Social Rehabilitation Counselor for Gilead’s Chester area adult social rehabilitation program works Monday – Friday 8:30AM-4:30PM (40 hours per week) providing case management, advocacy, and outreach in a program serving adults with major mental illness. The individual in this role carries out all phases of social support service including intake, engagement, assessment, treatment planning, case management, referral, and discharge. This role also requires the timely completion of required documentation including assessments and other treatment planning.
Minimum Requirements:
• Bachelor’s degree in a human service field
• One year of clinical experience
• Must maintain and provide proof of valid driver’s license and automobile insurance in good standing
• Reliable use of personal vehicle as needed
• Experience in the field of mental health and addictions required
• Previous social center experience preferred

This is a union position and pays $20.38 per hour dependent on level of education.
Responsibilities:
• Providing clinical knowledge regarding severe and prolonged mental health and substance abuse disorders through training and education
• Providing recovery-oriented case-management and social support services with an emphasis on dignity and respect through intervention and support, inclusion of family and community, and supportive counseling
• Providing socialization activities within the Social Center, following the International Community Clubhouse model of care
• Communicating with other care providers to coordinate care
• Monitoring positive outcome measures and ensuring compliance through accurate and timely data reporting
• Completing assessments, treatment planning documents, and other documentation electronically and on paper in a timely manner
• Acting with compassion, integrity, and ethics
