🏡 Full-Time RN Clinical Manager – Hospital at Home Program

📍 Location: Stuart to Vero Beach, FL (Hybrid)
🕗 Schedule: Monday–Friday, 8:00 AM – 5:00 PM
💰 Salary: $72,000 annually + Benefits

💼 Key Responsibilities:

• Manage patient intake, authorizations, and care planning for skilled and non-skilled services

• Conduct start-of-care and supervisory visits, as well as orientations and training for staff

• Provide clinical oversight aligned with best practices and agency policies

• Support and mentor field clinicians to ensure quality care delivery

• Maintain compliance with all regulatory and accreditation requirements

🎯 What You’ll Bring:

• Active RN license in Florida (required)

• Minimum 1 year of home care experience (preferred)

• Experience with Hospital at Home programs (highly preferred)

• Valid driver’s license and reliable transportation

• Strong communication, leadership, and time management skills

• EMR proficiency; AlayaCare experience a plus

• Positive attitude, flexibility, and commitment to professional excellence
