Position Summary
The Hospice of Baton Rouge is seeking a compassionate and experienced Full-Time Homecare RN Case Manager to coordinate and manage hospice care for an assigned patient caseload, providing comprehensive nursing assessments, individualized plans of care, symptom management, and patient and caregiver education. The RN Case Manager serves as the primary clinical coordinator, collaborating with the interdisciplinary team to promote patient comfort, dignity, and quality of life while ensuring compliance with agency policies and regulatory requirements.
JOB FUNCTIONS
- Perform comprehensive nursing assessments and develop, implement, and evaluate individualized plans of care based on patient needs.
- Provide skilled nursing care and symptom management consistent with hospice philosophy and standards of practice.
- Identify changes in patient condition, initiate appropriate nursing interventions, and communicate significant findings to the physician and interdisciplinary team.
- Coordinate medications, durable medical equipment, supplies, and other services necessary to meet patient needs.
- Provide emotional support and compassionate end-of-life care while promoting patient comfort, dignity, and quality of life.
- Respond appropriately to urgent patient care needs and participate in on-call responsibilities as assigned.
- Serve as the primary clinical coordinator for assigned patients and collaborate with physicians, nurse practitioners, facilities, pharmacies, and members of the interdisciplinary team to ensure continuity of care.
- Participate in admissions, transfers, discharges, and other patient care activities as assigned.
- Supervise Licensed Practical Nurses and Hospice Aides in accordance with state regulations and agency policy.
- Participate in Interdisciplinary Group (IDG) meetings and contributes to development and revision of patient care plans.
- Assist with bereavement follow-up and family support activities as assigned.
- Educate patients and caregivers regarding the hospice plan of care, disease progression, medications, symptom management, safety, and end-of-life expectations.
- Reinforce caregiver education throughout the patient’s hospice experience to promote confidence and quality care in the home setting.
- Complete accurate, timely electronic medical record documentation in accordance with agency policy and regulatory standards.
- Maintain compliance with CMS, CHAP, HIPAA, state regulations, and organizational policies.
- Participate in Quality Assessment and Performance Improvement (QAPI), competency validation, mandatory education, and survey readiness activities.
- Maintain current nursing license, certifications, and required continuing education.
Qualifications: Education:
- Graduate of an accredited school of nursing.
- Bachelor of Science in Nursing (BSN) preferred.
Certification
- Current, unrestricted Louisiana RN license required.
- Current BLS/CPR certification.
- Certified Hospice and Palliative Nurse (CHPN) certification preferred.
Work Experience
- Minimum of two (2) years of nursing experience in acute care, medical-surgical, oncology, geriatrics, home health, hospice, or a related clinical setting.
- Previous hospice or palliative care experience preferred.

Other Requirements
- Good physical assessment skills and computer skills.
- Good communication skills and techniques with facilities, patients, families, physicians, and office staff.
- Installation of company communication application on cellular device.
- Must be in good physical condition.
- Must have own transportation and provide proof of liability insurance which meet state requirements.
- Must be able to travel to and from all patient homes without assistance.
