Community Integration Specialist

32 Hours/Week | Flexible Community-Based Schedule

One Vision is seeking compassionate, dependable, and community-minded individuals to join our team as a Community Integration Specialist. In this role, you will support individuals with disabilities in participating in recreation, volunteer opportunities, and community activities that align with their personal interests and goals.

This position is ideal for someone passionate about helping others build independence, meaningful relationships, and strong connections within their community through person-centered support.

What You'll Do

  • Support individuals in participating in recreation, volunteer opportunities, and community activities aligned with their interests and goals
  • Promote independence, confidence, and meaningful community inclusion
  • Develop and adapt community-based activities based on individual needs and preferences
  • Build and encourage natural supports and community relationships
  • Ensure the health, safety, and well-being of individuals during all activities and outings
  • Provide transportation and support individuals in accessing community opportunities
  • Complete accurate and timely documentation in compliance with One Vision policies and regulatory requirements
  • Respond appropriately during emergencies and promptly report concerns
  • Maintain professional boundaries and model respectful interactions with individuals served, families, guardians, and coworkers

  • Strong attention to detail and documentation accuracy
  • Ability to adapt to changing schedules, environments, and individual needs
  • Moderate computer skills, including Microsoft Office, email, and internet-based systems
  • Ability to safely implement Mandt procedures in accordance with One Vision policies

Requirements

  • High School Diploma or GED required
  • Minimum of two (2) years of experience working with individuals with disabilities preferred
  • Valid driver's license with a clean driving record
  • Ability to perform physical tasks, including standing, bending, kneeling, reaching, and lifting up to 50 pounds occasionally
  • Successful completion of required background checks and health screenings
  • Completion of required certifications and trainings, including CPR, First Aid, and Mandt

Schedule & Flexibility

  • 32 hours per week
  • Variable and flexible hours based on individual needs and community activities
  • Non-traditional schedule that may include weekdays, evenings, and weekends as needed
  • Hours will vary depending on individualized supports and community opportunities

Safety Sensitive Position

This position is considered safety-sensitive. Employees in this role are responsible for supporting the health, safety, and well-being of individuals served and may operate vehicles, provide direct support, respond during emergencies, and assist with lifting and transferring responsibilities.

Successful candidates must be able to perform duties safely and in compliance with One Vision policies and procedures.
