We are seeking a compassionate and dedicated Case Manager who places trauma informed care at the center of their work and is committed to making a meaningful difference in the lives of children and families. This role is ideal for a professional who understands the impact of trauma, adversity, and disrupted stability, and is passionate about supporting youth and families in ways that promote safety, trust, healing, and resilience.
The Case Manager plays a key role in coordinating and delivering individualized services that support the unique strengths and needs of each child and family. Through consistent engagement, service planning, and collaboration, this role ensures care is strengths-based, culturally responsive, and centered on empowering families while promoting overall wellbeing and community connection.
In this role, you will partner closely with families, foster parents, and community providers to coordinate care, monitor progress, and advocate for the needs of youth. You will help guide families in accessing resources, navigating services, and creating stable, supportive environments where children can grow and thrive.
