THE CARE CENTER, INC.
Full-Time and Part-Time
Overall Function:
Provide support to clients/residents with mental health needs who have been incarcerated at the county, state, or federal level to integrate into transitional housing to offer a choice between custody and independence.
Essential Functions:
- Provides hands-on instruction to residents on implementing Individual crisis plans and assisting with crisis resolution.
- Provide support by helping clients live as independently as possible and reach attainable goals to maintain their independence.
- Aids and support with the resident's Activities of Daily Living and Instrumental Activities of Daily Living as indicated in the resident’s assessment and support plan.
- On a day-to-day basis, support with meal preparation, household chores, medication management, budgeting and finances, accessing community facilities/activities, shopping, socialization, doctor’s appointments, and providing emotional support.
- Assist the client with skill development and job placement.
- Monitor residents' progress and complete documentation, identifying problem areas that may need to be addressed.
- Participates in regular staff training, including specialized training to develop basic knowledge and skills to assist residents in crisis resolution, and support job performance.
- Responds appropriately to emergency situations.
- Maintains proper and detailed documentation of all assistance and support provided to each resident
- Other duties as required
- Adheres to all policies, laws, regulations, and codes of ethics and confidentiality as outlined by federal and state laws, and agency policies and procedures

Qualifications
- High School Diploma, GED, or an equivalent combination of skills, experience, and education may be considered at the discretion of the Agency.
- Ability to receive, understand, and communicate information effectively and follow instructions.
- Must have a valid Pennsylvania Driver’s License and a safe driving record.
- Must have the ability to utilize electronic medical records.
- Reliable, organized, and committed to the health and safety of the clients.
- Displays a positive and compassionate attitude and manages multiple tasks at one time.
- Must successfully obtain Criminal History Clearance from the PA State Police, Child Abuse Clearance from the PA Department of Human Services and FBI Clearance.
- Must not be excluded from participation in Medicare, Medicaid, or any other federal health care program.
- Ability to transport oneself and/or individuals in an insured vehicle, as program needs dictate.
- Ability to safely operate a variety of agency vehicles.
