HealthFirst Family Care Center, Inc. is a Federally Qualified Health Center located in Fall River, Massachusetts. For over 50 years, HealthFirst has provided comprehensive, high-quality, and culturally responsive care to residents of the Greater Fall River area. HealthFirst’s services include adult and pediatric primary care, behavioral health, oral health, care management, and limited specialty services such as gynecology, addiction medicine, and Hepatitis C care. We also operate school-based health services and offer integrated nutrition and dental outreach programs. Our team of 175 + professionals serve a socioeconomically and culturally diverse population of 14,000 patients annually.
Program Overview
HealthFirst’s Family Nurse Practitioner (FNP) Residency is a paid twelve-month, post-graduate program launching in September. Designed to provide continued support after twelve months to new nurse practitioners as they transition into independent practice, the program emphasizes culturally responsive, community-based primary care for complex and underserved populations across the lifespan. Through structured mentorship, specialty rotations in women’s health, pediatrics, adult medicine, obesity, Hepatitis C, behavioral health, and addiction medicine that strengthen comprehensive primary care skills, and hands-on clinical experience, residents build the clinical expertise, operational confidence, and professional judgment needed to thrive in a primary care setting. The residency is followed by a one-year employment commitment, offering a valuable opportunity to further refine skills in a full-time nurse practitioner role.
Clinical Training & Educational Experience
Each resident is precepted by an attending provider for the full year to ensure strong supervision and continuity of care. Residents work alongside experienced preceptors and interdisciplinary team members with diverse expertise. The curriculum includes weekly didactic sessions, daily primary care support, and frequent shadow sessions with experienced providers in specialty rotations. Learning is further strengthened through extensive orientation, structured mentorship, routine feedback, and completion of a Quality Improvement Project. There are protected hours for didactic, support, and mentorship sessions.

HealthFirst Family Care Center, Inc. in Fall River, MA began as a Model Cities Program in 1971 in response to a need for accessible medical and dental care expressed by residents. The City of Fall River provided strong support for this program and its name was subsequently changed to the Community Development Service Center.
The Center has served three generations of families. HealthFirst’s service area has grown over the past 50 years to include the Portuguese, Hispanic, Cambodian, African American and Brazilian Portuguese populations.
