Our Floating RN Visit Nurse position was built differently. Instead of permanently assigning you to one territory, we'll deploy you to an area of our service region where additional nursing coverage is needed. You'll typically spend approximately three months in that area —long enough to learn the territory, establish relationships, understand the patient population, and become part of the clinical team. Then, when needs shift, so do you. New territory. New patients. New team. New challenges. Same nursing expertise. This Isn't Random Daily Floating. Let's make that distinction clear. You aren't waking up every morning wondering which side of the Twin Cities you're driving to. Our floating model is designed around extended territory assignments , typically lasting around three months at a time . During each rotation, that becomes your primary coverage area. You'll have the opportunity to settle into a rhythm, learn the geography, get to know your patients, and work closely with the team you're supporting. Then you'll eventually transition to another part of our 21-county service area where your skills are needed. You get the variety of floating without changing territories every day. Think of It as Seeing More of Home Health. A traditional Visit Nurse can become an expert in one territory. A great Floating Visit Nurse becomes comfortable in all of them. Over time, you'll experience different patient populations, communities, referral patterns, clinical teams, and coverage needs. You'll learn how different parts of our service area operate. You'll build relationships throughout the organization. And you'll develop the confidence to enter a new territory and quickly figure out how you can help. You don't simply get handed an address and figure everything out yourself. You'll step into a territory with a purpose: Provide additional nursing coverage where our patients and teams need it. You'll begin learning the geography. Getting familiar with the patients. Working with the clinicians supporting that area. Understanding where the greatest needs are. And before long, what started as a new territory begins feeling familiar. That's when you'll be doing some of your most important work—providing the consistency and coverage that helps keep patient care moving. What You'll Be Doing Provide skilled RN home health visits within your assigned coverage area Rotate throughout different portions of our service area based on patient volume and staffing needs Typically remain within each coverage territory for approximately three months Complete comprehensive nursing assessments Perform wound care, medication management, injections , and other skilled nursing services Monitor patients with acute and chronic medical conditions Identify and communicate changes in condition Educate patients and caregivers on medications, diagnoses, treatments, and recovery Coordinate care with physicians and healthcare providers Collaborate with PT, OT, SLP, social work, and interdisciplinary team members Help maintain continuity of care during changing census and staffing needs Complete timely documentation in Homecare Homebase (HCHB) This Role Rewards Adaptability. Who We're Looking For Active Minnesota Registered Nurse (RN) license Previous experience in home health, hospital, acute care, rehabilitation, skilled nursing, or another clinical environment Strong clinical assessment and critical-thinking skills Confidence providing care independently in patients' homes Strong organization and time-management abilities Excellent patient, caregiver, and team communication Flexible and adaptable approach to changing assignments Ability to quickly become comfortable within new territories Reliable transportation Willingness to travel throughout our 21-county service area Previous home health nursing experience is especially valuable given the independence and adaptability required for this position.
