The CARE Case Manager coordinates routine and crisis case management for the WFU students, faculty, staff, and community. This position serves as a member of the WFU CARE Team. Additional responsibilities include community education and outreach, data collection, relevant special projects, and other general case management duties for the Office of the Dean of Students in collaboration with University and community partners.

Essential Functions:
Provide case management services for students, staff, and faculty experiencing personal, medical, or mental health-related crises. This includes helping individuals become connected with on and off-campus support resources, communicating appropriately with all parties involved, and engaging in timely documentation.
Serve as one primary point of contact for the WFU community regarding. persons in need of caring outreach and/or case management coordination).
Maintain case notes containing sensitive information and provide accurate and thorough documentation of weekly Team meetings, emergency meetings, as well as daily case management activities throughout the week via electronic record-keeping.
Provide support for CARE Team activities (e.g., weekly and ad hoc meetings).
Collaborate with campus partners, departments, schools, and colleges to achieve case management goals.
Required Education, Knowledge, Skills, Abilities:
Master’s degree in Social Work, Mental Health, Human Services, Higher Education, Counseling Divinity, or related field and two years of relevant work experience.
Excellent interpersonal communication skills; ability to communicate effectively with groups and work and communicate well in a team environment.
Ability to assess information regarding identified needs for case management, caring outreach, and persons of concern.
Ability to professionally and appropriately manage sensitive, private material and information.
Knowledge, experience, and comfort with recognizing and supporting individuals experiencing mental health issues and symptoms.
Knowledge of risk factors and warning signs related to the potential for targeted violence on campus and within the community.
Ability to synthesize and summarize large amounts of information, document information electronically in a timely manner, and verbally communicate with appropriate stakeholders as warranted.
In times of crisis intervention, work may require night and weekend availability.
Knowledge of WFU and community resources, ability to communicate effectively to facilitate referral to those resources.
Ability to tolerate, convey, and protect sensitive information.
Understanding of and ability to adhere to strict privacy restrictions.
Skill in using technology to track and manage individual cases as well as assess and report on campus trends.
Knowledge of current regulations relevant to work in higher education (i.e., Title II, III, & IX; FERPA; HIPAA; etc.).
Preferred Education, Knowledge, Skills, Abilities:
Three to five years of relevant work experience.
Experience with case management in organizations, specifically in higher education.
Accountabilities:
Responsible for own work.
Physical Requirements:
Sedentary work that primarily involves sitting/standing.
Occasionally moving objects up to 20 pounds.
Environmental Conditions:
