About the Role

The Field Supervisor plays a key role in ensuring clients receive safe, high-quality care in their homes. • Conduct 2 hours of on-the-job training for each new Personal Assistant (PA)

Caregiver Oversight

  • Schedule and perform in-field caregiver evaluations
  • Recognize and reinforce strong performance; identify areas needing improvement and provide coaching, in coordination with the Program Manager

Quality Assurance

  • Conduct monthly quality assurance calls/visits to monitor client status, care quality, and caregiver adherence to the Code of Ethical Behavior, dress code, and documentation standards
  • Investigate client complaints or incidents (e.g., property damage, caregiver concerns, non-compliance) in coordination with the Program Manager, and help implement action plans

General Support

  • Assist with general office tasks as assigned
  • Follow departmental policies, safety protocols, and infection control measures
  • Perform other duties as assigned by the Program Manager

What You'll Need

Courage – With an optimistic and brave heart we face each challenge, celebrating successes and viewing failures as opportunities to grow.

Community – We are active participants in our communities…reaching out to be good neighbors.

Our Story

The story of the legend of the phoenix bird is a story of ever-changing growth and rebirth. The beautiful mythical creature reflected the lights and colors of the morning sunrise – – always chasing the light and capturing all that was beautiful. It is a symbol of constant rebirth, renewal and fresh starts. It is not a story of ashes but rather of beauty and upward thinking…(Isaiah 61:3).

Our energy is focused on improving quality of life, perpetuating positive attitudes and embracing the challenges life places before us with grace, respect and compassion.

Our catalyst to consistently rise to new heights is a combination of perpetual learning, measurement of success, looking through the eyes of those we serve and listening with the heart which causes us to pause and discover what is really important in each situation. Within the listening and the application of learning we find new beginnings.

…We are Phoenix Home Care.
