$7,500 Sign-on Bonus!

Monday-Friday 8:00am-5:00pm

No On-call, no weekends and no holidays required!

The Optum SCC program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of members. The Optum SCC SL RN will be managing members in a community setting to include personal homes, group homes, and assisted living facility setting. Primary Responsibilities:

  • Ensure accurate and complete documentation
  • Communicate and collaborate with the interdisciplinary care team
  • Support the telehealth visits in the field, the RN sets up members for telehealth with the APC
  • Assist the provider / team with various care coordination activities
  • Assist the provider / team with benefit determination associated with Medicare part A, part B, and part D benefits
  • Responsible for collaborating with the primary advanced practice clinician (APC) and facility to identify and respond to patient change in condition
  • Assist the provider / team with therapy coordination
  • Participate in facility-based meetings
  • Schedule and participate in family conferences, team meetings, and team case presentations
  • Coordination of training, tracking, and compliance with quality measures
  • Assist in creating and conduct in-services for selected audiences
  • Assist Providers in assuring complete and accurate documentation and coding, medication reconciliation, nursing rounds, assessment, and patient documentation.
  • May write verbal orders from Provider in the chart in compliance with the state RN practice laws, and in compliance with individual nursing facility practices
  • Assist in managing members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventions
  • Assist APC in obtaining compliant related documentation for transfer to stay in a SNF

Required Qualifications:

  • Active and unrestricted RN license in OR
  • Associate degree or higher in Nursing (RN)
  • Certified in Basic Life Support
  • 3+ year of clinical experience in practice
  • Ability to complete physical requirements, including moving a 30-pound bag and ability to perform physical assessment
  • Access to reliable transportation and willingness to travel in between member locations daily
  • Proficient computer skills, including the ability to document medical information with written and electronic medical records
  • Proficient in Microsoft Office products including Word, Excel and Teams
  • Ability to develop and maintain positive customer relationships
  • Highly adaptable to the changing needs of our members daily
  • Effective communication skills and time management

Preferred Qualifications:

  • 1+ year experience working in long-term care, home health, hospice and/or case management
  • Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
  • Understanding of older adults and chronic illness
  • Understanding of Advanced Illness and end of life discussions
  • Ability to work with diverse care teams in a variety of settings

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $60,200 - $107,400 annually based on full-time employment. We comply with all minimum wage laws as applicable.
