In this role, you will provide person-centred support to a client with Acquired Brain Injury, Schizophrenia and Chronic bowel and bladder incontinence - assisting with daily activities, encouraging independence, and building meaningful connections.
Key Responsibilities:
Personal hygiene prompting and supervision
Continence management
Safe showering and dressing
Medication supervision
Meal preparation and nutritional monitoring
Appointment attendance and community access
Transfer and mobility assistance
Monitoring of respiratory and physical health status
Environmental safety checks
Emotional regulation support
Routine establishment and wind-down
Monitoring of mental state stability
Requirements:
Relevant experience supporting clients with:
Acquired Brain Injury
Schizophrenia
Chronic bowel and bladder incontinence
Type 2 Diabetes Mellitus
Certificate III or IV in Individual Support (Disability) or equivalent
Current First Aid & CPR certificate
Valid Australian Driver’s Licence and reliable vehicle
NDIS Worker Screening Check/Yellow Card
Strong communication and interpersonal skills
