Job Shift: DayHeartShare St. Vincent’s Services is currently seeking a Family Team Conference (FTC) Facilitator for our Foster Care program. The Family Team Conference (FTC) Facilitator is responsible for guiding and tracking all conferences as required by the Administration for Children’s Services (ACS) utilizing the Family Team Conference Model. The facilitators organize and lead various types of Family Team Conferences, such as service plan reviews, foster placement stabilization meetings, and discussions about permanency planning goals. Utilizing strong engagement skills, the facilitator is expected to yield positive safety and permanency outcomes for children and families with the input from a multidisciplinary team and various stakeholders. This position also supports engagement with families by facilitating conferences, assisting with Power of Two referrals and referrals that strengthens permanency outcomes.
