Summary:

Employee is a trained home visitor (Para-Professional) for the HANDS program in the state of Kentucky. Credentials will be kept on file and updated on a yearly basis or as changes arise. The pay scale range will be $13.50 - $20.07 per hour/75 hours bi-weekly. A cell phone reimbursement of up to $75 per month will also be provided. The workstation will be Morgan County Health Department with travel outside Morgan County determined by District caseload needs. Travel will be reimbursed at the approved state rate.

Description:

HANDS Services:

1. Provides ongoing HANDS services to first time and multigravida families to support identified HANDS Goals.
2. Provides regular planned visits to establish a trusting relationship for voluntary eligible HANDS participant(s) to assist parent(s) and/or families with child development, parenting skills, health services and other needed resources identified as assessment or during visits.
3. Provides ongoing Resource Referral and facilitation of access to other services provided by Community partners/health department that are outside the scope of services approved by HANDS (family planning, mental health services, intimate partner violence services, daycare, prenatal/pediatric health services, Head Start, Substance abuse services, education and employment, and Family Preservation.
4. Completes and submits required HANDS documentation and billing per required policy specifications and timelines. (Home Visit Logs, Supplemental Billing, Forms/Tools (EDS, DV, ASQ, CPCL, etc.), as well as Corrections)
5. Participates in Ongoing Home Visitor Supervision per required HANDS Policy.
6. Provides weekly completed number of visits from previous week and weekly schedules to supervisor.
7. Maintains confidentially of program information obtained from home visit(s) and exchanged while communicating with community and with support providers.
8. Completes yearly review and signature of the HANDS Code of Ethics and Home Visitor Safety materials per policy requirements.
9. Maintains strength-based communication within the workplace, with community partners, with HANDS team and Program Management and with HANDS families.
10. Confers with Program Coordinator and/or Supervisor in cases involving potential collaboration with other service providers. (Ex. Child Protection, legal, intimate partner violence)
11. Completes and documents required training on an Individual HANDS Training Log including Tier II Premium.
12. Maintains required Productivity expectations, per HANDS policy, based on the weighted caseload, not number of individual families. A full weighted caseload requires 60 visits per month for a full-time home visitor.
13. Productivity: Employee must meet or exceed HANDS policy standards regarding productivity for the position description noted above. Productivity below HANDS policy standards will result in corrective actions being taken to support employee in meeting set productivity standards. Continuous productivity below HANDS policy standards for timeframe of 6 months may result in termination from contract position.
14. Demonstrate fidelity regarding the delivery of Growing Great Kids materials.
15. Attends team meetings and in-service training as required by program protocol.
16. Performs other duties, as required that are reasonable and applicable to this position as assigned by the Supervisor. (Ex. Sub-visits)
17. Refrains from use of the GGK curriculum materials outside their licensed agency.
18. Will leave curriculum materials with the licensed organization should either party end this employment agreement.

• Must be 18 years of age. Must have telephone accessibility. Must have reliable transportation and valid Driver’s License. Criminal background check.

POST EMPLOYMENT REQUIREMENTS: EMPLOYEES IN THIS CLASS WILL BE REQUIRED TO OBTAIN CREDENTIALS OUTLINED BELOW WITHIN A REASONABLE TIME, OR IF EMPLOYEES POSSESS SUCH CREDENTIALS AT THE TIME OF HIRE THEY WILL BE REQUIRED TO MAINTAIN THOSE CREDENTIALS SO LONG AS THEY ARE EMPLOYED IN THIS CAPACITY.

• On the job training requirements needed within 6 months of hire in order to advance: HANDS home visitor core training (Must be completed before family contact can occur), HANDS wrap-around required training (Specific topical areas checklist), CPR Certification, Working knowledge of geographic area and or ability to locate participants and community resources with use of maps, contacts, and directions provided.

• Conduct initial home visit following Parent Visitor assessment with first time parent/s and or families to establish rapport and planning for providing services as qualified by the provider. Provide regular planned visits to establish a trusting relationship for voluntary eligible HANDS participant/s to assist first time parent/s and or families with child development, parenting skills, health services and other needed resources identified at assessment or during visits. Facilitate families in securing of prenatal/child health services, referrals, and appointments with other supportive agencies. Document case records, home visit logs accurately on each contact, progress, service need and completes reports as required. Maintain confidentiality of program information obtained from home visit/s and exchanged while communicating with community and support service providers. Confer with Program Coordinator and/or Supervisor in cases involving potential collaboration with other service providers.

TYPICAL WORKING CONDITIONS AND UNIQUE PHYSICAL REQUIREMENTS: INCUMBENTS IN THIS CLASSIFICATION WILL TYPICALLY PERFORM THEIR PRIMARY JOB DUTIES UNDER THESE CONDITIONS. THESE CONDITIONS MAY CHANGE ON OCCASION IN PERFORMING THE DUTIES OF AN INDIVIDUAL POSITION.
