- Responsible for assessing and treating mental health, substance use, or co-occurring disorders to children and families in the community on the Family Support Team serving Charlotte County.
- Complete thorough comprehensive assessments for adults and children, as scheduled and/or needed.
- Development and implementation of recovery-focused service plans, which include measurable objectives stated in appropriate terms.
- Provide evidence-based brief and solution-focused individual and family therapy.
- Maintain accurate and timely documentation, per agency standards.
- Provide individualized services in the community, including in their homes and other community settings.
- Carries a caseload and provides case management services to patients as needed as part of a multidisciplinary team.
- Attend and participate in the Team Clinical Staffing and treatment planning meetings twice weekly.

Qualifications
- Master’s degree in a Human Services field.
- Minimum one (1) year working with children and / or families.
- Ability to pass a level II Background clearance and drug test.
- Valid FL drivers’ license, insurance, and safe driving record.
- Dependable transportation (registered, safe operating conditions, etc.).
