Salary: $25.72 - $38.58 Depending on Qualifications
JOB PURPOSE: The Family Support Specialist (FSS) is responsible for providing home/community-based prevention services focusing on supporting the parent child relationship, attachment, and family enhancement through child development education, play, goal setting, assessments, and coordination of services/activities for overburdened families. The FSS works in collaboration with the patient care team in supporting improvement of patient care and their quality of life.
The primary goal of the El Rio Health Family Support Specialist is to support El Rio’s Mission of providing comprehensive, quality health care that is affordable and accessible to all who may have healthcare needs, by successfully performing the primary essential functions.

Job Functions:
• Performs and/or completes all assigned duties, tasks, responsibilities, and assignments supporting program operations within established periods; meeting established rates of performance for the quality and quantity of work for the position; demonstrating a level of quality, efficiency, and accuracy in the employee’s job performance that ensures the highest standards of excellence.
◦ Always maintains employee and/or patient confidentiality by controlling the environment and information being disclosed only to authorized individuals ensuring compliance with all HIPAA and corporate compliance standards.
◦ Understands and complies with all organizational Code of Conduct standards, Policies and Procedures, and Service Excellence requirements with all patients, visitors, employees, and internal/external clients or representatives.
• Performs assigned duties, tasks, or assignments that support effective family support to patients, such as:
◦ Conducts home visits virtually or in-person and assesses family needs and provide case management services, when necessary, while promoting self-sufficiency.
◦ Utilizes child development curriculum with each family and promotes positive parent-child relationships.
◦ Utilizes child appropriate activities to support the parent and child relationship.
◦ Utilizes appropriate limit setting and boundaries.
◦ Acts as a positive role model.
◦ Provides advocacy, emotional support, and help in problem-solving. Family support services will be offered in the office but primarily in the community to meet families where they are at and may be offered after hours and on weekends.
◦ Helps to locate support services in the community that can assist in navigating the public health system.
◦ Helps guide children and families in the development of their own family goals.
◦ Encourages communication with the multidisciplinary integrated treatment team.
◦ Works with the multidisciplinary integrated treatment team members in a manner that supports the goals of children and families.
◦ Facilitates family support groups and/or classes.
◦ Serves as a treatment team member and participates in team meetings.
◦ Works closely with the Behavioral Health Leadership Team
• Receive clinical oversight and supervision by a Behavioral Health Professional at least once every two weeks as specified by regulatory requirements.
• Demonstrates an exceptional level of customer service; answers and responds to incoming calls, emails, and inquiries in a timely and effective manner, responds to requests for support providing general information in response to inquiries.
• Identifies and reports any safety hazard, or potential hazard, by immediately notifying supervisory staff and communicating the hazard, thereby ensuring that El Rio Health provides safe and high-quality services for all patients and visitors.
• As a team player, demonstrates self-initiative by being able to communicate effectively to assist patients, co-workers, and others while maintaining and enhancing a respectful and positive attitude in the work environment.

Joint Commission Responsibilities:
• Maintains a clean, safe, and hygienic work environment in compliance with all Policies and Procedures including but not limited to work areas, workstations, examination rooms, hand washing, infection prevention and control etc. for this position.
• Demonstrates an understanding of and proficiency with the application of all compliance and reporting requirements respective to Joint Commission Certification (JCC) standards.
Other Duties and Responsibilities:
• In support of the Mission and Vision of El Rio Health, will at all times represent themselves as a professional role model of El Rio Health serving as a positive informational source for members of the community.
• Supports El Rio Health by participating in community events which promote good health, and which contribute to a broader awareness and understanding of El Rio Health and the many services provided to the community.
• Participates in all appropriate health care activities related to El Rio Health on a local, state, and national level to maintain and enhance El Rio’s image with health agencies, professional associations, and the general public.
• Ensures and supports the cost-effective use of materials, supplies, and equipment by limiting waste of all organizational supplies and resources.
• In accordance with policies and procedures ensures that documentation of hours worked is completed accurately, as well as signed-off/approved in a timely manner.
• Employees must maintain their skills proficiency with El Rio Health operating systems and/or software programs by adapting to changing requirements and successfully learning and demonstrating new skills in response to software, system, and program improvements or upgrades.
• Performs other duties as assigned.
Qualifications:
If applicable, equivalent combination of education and experience may be considered, and must be directly related to the functions and responsibilities of the job.

• Employees in this position are required to have reliable transportation that can meet any operational reassignments of the organization during the workday.
