Are you passionate about supporting your mob and making a difference in the lives of Aboriginal and Torres Strait Islander children and families?

DYHS is looking for a Family Support Advocate to join our lovely Derbarl Kids team!

As our Family Support Advocate you will be working alongside the Paediatric team in improving the health, learning and social wellbeing of our children.

This role will work within our multidisciplinary team including the Paediatricians, Nurses, GPs and Allied Health. This team strives to provide high quality, comprehensive, culturally safe clinical care to clients and maintains a focus on best practice family-centred and culturally appropriate care.

This role will be a Maximum Term Full-Time contract which expires 12 months from the start date.

About the Organisation

The Derbarl Yerrigan Health Service (DYHS) is the largest Aboriginal community-controlled health service provider in Western Australia, providing care to over 20,000 Aboriginal patients across four clinics in East Perth, Mirrabooka, Maddington and Midland.

Key Responsibilities

• Build and nurture strong relationships with children and families who access our services.

• Help our families with completing forms and attending appointments.

• Support and build their capacity to navigate external services - e.g. Centrelink, NDIS, Dept. of Communities.

• Maintain accurate and confidential client records.

• Work in a multidisciplinary team and assist with intake and case management.

• Ensure all work is conducted in a culturally safe and family centred manner.

Who we're looking for:

• A person of Aboriginal and/or Torres Strait Islander descent.

• Someone with a warm, respectful approach.

• Genuine care for mob and connection to community.

• Completed post-school qualification in a relevant field (Health, Social Work or equivalent).

• Competency with Microsoft Office and client management systems.

Appointment Pre-requisites

Provision of the minimum identity proofing requirements

• Successful Criminal Record Screening Clearance

• Successful Working with Children Check

• Evidence of a current C or C-A Class driver’s licence or other specialised licence class
