We have an exciting opportunity for a Family Service Worker to join our team of talented professionals.

The Family Service Worker will work as a member of a team with program staff and community resources to provide a continuum of services to children and families in an effort to strengthen and preserve families, assist in identifying individual needs and providing and assisting in coordinating services to meet each family's needs.

Specific responsibilities include:

1. Assist families in completing required enrollment, eligibility, and intake documentation.
2. Develop and monitor Family Partnership Agreements with parents to identify strengths, needs and goals.
3. Provide referrals for community resources including housing, employment, education, and health services.
4. Maintain accurate documentation, case notes, and follow-up activities in accordance with program standards.
5. Support parent engagement activities, including parent meetings, workshops, and events.
6. Use data management system to track family progress, case notes and health data.
7. Monitor children’s attendance and support families in addressing attendance barriers.
8. Assist in the recruitment and enrollment of children and families.
9. Establish and maintain relationships with community partners to expand the resources available to families.
10. Participate in annual program self-assessment, training, and ongoing professional development.
11. Participate in multidisciplinary team meetings, include self assessments and continuous improvement activities.
12. Perform other duties as assigned.

Working hours are 37.5 hours per week.

Requirements

Qualifications: Bachelor’s degree in social work, human services, family studies, family services credential or related field required; Experience working with families in a social services, education, or community-based program. Knowledge of community agencies and family support resources. Strong communication, documentation, and case management skills. Ability to maintain confidentiality and professionalism.
