Purpose of the Position

The Facilitator’s primary role is to support the Team Leader to achieve client engagement and client outcomes.

Responsibilities and Duties

  • Manage and coordinate the family led decision making meetings process including referrals, identifying, and coordinating participants, and facilitating meetings.
  • Ensure the family led decision making process is facilitated in accordance with legislative principles and requirements.
  • Prepare people to participate in family meetings, including ensuring that all participants have a clear understanding of the purpose of the family led meeting.
  • Obtain the views of persons who are unable to attend the family meeting.
  • Manage conflict that may occur within the family led decision making process.
  • Liaise with relevant internal staff and the department
  • Assist families in developing family led plans for submission to the department.
  • Maintain accurate records of case plans and other relevant information in accordance with departmental guidelines.
  • Develop and maintain positive and professional relationships with referral partners and other service providers
  • Participate and contribute to the organisation’s compliance obligations under the ISO 9001:2015 Quality Management System and Human Services Quality Standards (HSQS)
  • At all times work within CQID’s Workplace Health & Safety approach, Policies and Procedures
  • Other responsibilities as directed by the Team Leader/Manager

Qualifications and Experience

  • Holds a current C Class Queensland Driver’s Licence
  • Tertiary qualifications in Human Services
  • Current Working with Children Check, “Blue Card”
  • Qualifications or knowledge related to Child Protection
  • Holds or is willing to obtain a First Aid and CPR Certificate
  • Experience at successful service delivery to Aboriginal and Torres Strait Islander people
  • Experienced in client engagement and case collaboration

Skills and Attributes

  • Aboriginal and/or Torres Strait Islander heritage
  • Experience in developing and managing case plans
  • Competent in use of Microsoft Office software
  • Physical ability to undertake job responsibilities
  • Ability to communicate effectively; spoken and written
  • Self-starter, motivated and able to self-manage work tasks
  • Strong connection to and knowledge of local Indigenous Community
