Family Nurse Practitioner - House Calls
Location: Brooklyn, NY
Compensation: $135K+ (Based on Experience)
We are hiring a Family Nurse Practitioner (FNP) to deliver in-home healthcare services to patients throughout Brooklyn. This is an excellent opportunity for an FNP seeking a rewarding career in primary care, geriatrics, or home-based medicine.
Conduct patient evaluations, physical exams, and health screenings in a home setting
Diagnose and manage acute and chronic conditions
Prescribe medications and develop personalized care plans
Collaborate with a multidisciplinary team to ensure the best patient outcomes
Provide preventative care and patient education
