Family Nurse Practitioner (FNP) – Community Health Center – Woodsville, NH

The Position:

We are seeking a board-certified Family Nurse Practitioner for outpatient primary care in Woodsville, NH. Our Client offers a structured 12-week onboarding program, provider mentorship, and meaningful input into organizational decisions. Salary and Benefits:

• $114,000 to $120,000 Annual Salary
• Negotiable Sign-On and Retention Bonus (Typically $5,000 Paid in Installments)

Requirements:

• Active New Hampshire APRN License (or eligible)
• Board Certified as a Family Nurse Practitioner (FNP-BC or FNP-C)
• 3 Years of Direct Primary Care Experience Preferred
• Experience with Electronic Health Records (AthenaOne Preferred)

Responsibilities:

• Monday through Friday Schedule
• No Weekends and No Call
• Approximately 80% Clinical and 20% Administrative Time
• Approximately 2 Patients per Hour
• Provide Outpatient Primary Care to Patients Across the Lifespan
• Manage Preventive Care, Chronic Conditions, and Acute Visits
• Order and Interpret Labs and Diagnostic Studies
• Prescribe and Manage Medications
• Collaborate with an Integrated Team of Physicians, Advanced Practice Providers, and Behavioral Health Professionals
• Participate in Provider Mentorship, Quality Initiatives, and Organizational Decision-Making
• Complete Timely Documentation in AthenaOne
