Full-Time | Outpatient Clinic | No Nights, Weekends, or Call

CentraCare – Clearwater is seeking a Family Medicine Advanced Practice Provider (NP or PA) to join our collaborative primary care team. You will provide comprehensive, team-based care that supports the Quadruple Aim — improving patient outcomes, enhancing patient and provider experiences, and ensuring cost-effective care.

Your Schedule

• 8 – 1.0 FTE (flexible based on your needs)
• Choose a 4- or 5-day workweek
• Clinic hours:
  • Monday: 7:30 a.m. – 5:00 p.m.
  • Tuesday–Friday: 8:00 a.m. – 5:00 p.m. (with flexibility as needed)
• No evenings, weekends, holidays, or call

Your Role

As a trusted primary care provider, you will:

• Diagnose and manage acute and chronic conditions
• Deliver preventative and wellness care, including routine checkups, screenings, and immunizations
• Provide personalized health counseling and education
• Coordinate care across specialties for comprehensive patient support

What We’re Looking For

• Graduate of an accredited Nurse Practitioner or Physician Assistant program
• 1+ year of clinical experience as an NP or PA preferred
• Current or eligible Minnesota licensure
• Active AHA BLS certification and DEA registration (or ability to obtain prior to start)

Experience in any of the following settings is a plus:

• Family Medicine
• Internal Medicine
• Urgent Care
• Emergency Medicine
• Pediatrics

Pay and Benefits

• Starting pay begins at $109,241.60 per year; exact wage determined by years of related experience
• Salary range:  $109,241.60 - $124,904 per year
• Salary and salary range are based on a 1.0 FTE, reduced FTE will result in a prorated offer rate
