Our team is dedicated to providing high-quality, patient-centered care both in-clinic and remotely.
Position Summary We are seeking a motivated and detail-oriented Medical Assistant (MA) to support our Remote Patient Monitoring (RPM) program. This role focuses on tracking patient health data, engaging patients, and ensuring timely communication with providers to improve chronic disease management. Key Responsibilities - Monitor daily RPM alerts (blood pressure, glucose, weight, etc.) - Review and document patient readings in the EMR - Contact patients regarding abnormal readings or missed data - Provide basic patient education on device use and compliance - Escalate clinical concerns to providers appropriately - Assist with onboarding patients into the RPM program - Ensure accurate and timely documentation - Collaborate with clinical staff to support care plans
Qualifications
