Family Medicine Advanced Practice Provider (Nurse Practitioner or Physician Assistant)

*Well-established practice with a supportive team and a new clinic space currently under development*

Location: CentraCare – Litchfield Clinic
FTE: 1.0
Schedule: Monday–Friday, no weekends, no call, no holidays

CentraCare is seeking a full-time Family Medicine Advanced Practice Provider to join our Litchfield clinic team. In this role, you will work with an existing patient panel and provide comprehensive care to patients of all ages, including acute and chronic care management, preventive services, routine checkups, screenings, immunizations, and personalized lifestyle counseling.

Responsibilities:

• Independently evaluate and treat patients for acute and chronic conditions
• Deliver preventive care and routine screenings
• Provide personalized counseling to promote healthy lifestyles
• Coordinate care for patients with chronic illnesses

Requirements:

• Current NP certification or Minnesota Physician Assistant licensure (or eligibility)
• Current American Heart Association BLS Healthcare Provider card
• DEA registration prior to receiving clinical privileges

Preferred (not required) Experience:

• Family Medicine
• Emergency Medicine
• Urgent Care

Pay and Benefits

• Starting pay begins at $109,241.60 per year; exact wage determined by years of related experience
• Salary range:  $109,241.60 - $124,904 per year
• Salary and salary range are based on a 1.0 FTE, reduced FTE will result in a prorated offer rate

• Staff of multi-specialty Physicians and APPs in the areas of Family Medicine, Internal Medicine, and General Surgery
• Campus includes two clinics and Meeker County Memorial Hospital, a 38-bed facility providing a range of primary and emergency care services as well as many support programs
• Outreach services provided by specialists from Willmar campus and other specialty health partners in Orthopedics, Oncology, Dermatology, Podiatry, Cardiology, Audiology, Certified Diabetes Educator, Certified Dietician and RN
