Salary Range: $50,000.00 - $70,000.00 Salary/year
Travel Percentage: Up to 10%The Buckeye Ranch is seeking a Family Liaison to join our Residential Clinical team. The Family Liaison will provide referral review for clinical acceptance, preadmission case management and preadmission contacts to family, youth and child and family team members, completion of diagnostic assessment to include diagnosing and completing necessary information in electronic health records to facilitate proper billing of services.  Warmly hands off admission to service team to ensure continuity of care. Post admission, the Family Liaison will provide direct case management services and support for youth and families and will also provide service coordination as needed. The position will help to determine and maintain an optimal level of family involvement by identifying and facilitating opportunities to engage family members in the treatment process. The Family Liaison will help raise the voice of the family. This position will follow up with coordinating entities to ensure discharge and aftercare planning which includes identification of a discharge plan and building a plan for aftercare services that will promote stability for youth after discharge from the Residential Services. Completion of aftercare communications and customer services post discharge.

QualificationsWho you are:
- You have a bachelor’s degree in Social Work, or related field; masters preferred.
- You have a year of relevant experience.
- You have an active Ohio LSW or LPC.
- You have an active driver’s license and proof of auto insurance.
