Job Description
of Performance Expectations:
Provides counseling and guidance support to patients and families throughout the hospital as needed. Assess and evaluate discharge planning needs in collaboration with physicians, direct care nurses, Patient Care Managers/Discharge Planners/Social Worker, and associated clinical professionals. Participates in interprofessional team meetings. Assists patients and families in obtaining financial assistance, psychosocial support, community resources, advance directive education, and medical equipment. Works closely with community agencies that provide services and resources to patients and families. When applicable, facilitates support groups and collects data required for accreditation programs.
Education & Credentials:
Required
Master’s degree in Human Service Field (Counseling, Sociology, Psychology). May consider a Master’s degree with five or more years of experience with mental health counseling.

Required
Experience in acute care discharge/transition of care planning associated with social needs identified based on social assessment. Given training and on-the-job experience, incumbent should be proficient in the basic aspects of the job within three months of employment date.
