<div>Title: Family-Based Provider Liaison<br>Reports To: Executive Director<br>Classification: Part-Time<br>FLSA Status: Non-Exempt<br><br>JOB SUMMARY<br>The Family-Based Provider Liaison is responsible for recruiting and training new family-based providers for adults and children. Host and attend events and create marketing material to recruit new families. Be certified to train family-based providers in all areas and to complete all required paperwork.<br><br>ESSENTIAL JOB FUNCTIONS<br>● Supervise staff and schedule them for their assigned training and paperwork.<br>● Relay any concerns or problems with Family Base Providers to the Executive Director.<br>● Function as a liaison for family-based providers, DCS, and the Program Coordinator.<br>● Interview prospective individuals served and families.<br>● Coordinate and schedule appointments, meetings, and related activities.<br>● Attend all required meetings (staff, COS, PCSP, and training).<br>● Serve as a support for the individual served, their family, and the Evergreen staff who provide services.<br>● Ensure that all policies, procedures, regulations, and guidelines are followed.<br>● Complete home study in compliance with timelines.<br>● Work flexible hours as requested by the Program Coordinator or Executive Director.<br>● Evaluate Family-based Providers as needed.<br>● Participate actively in contracting with family-based providers.<br>● Conduct unannounced visits to facility sites as directed by the Program Coordinator or the Executive Director.<br><br>CORE COMPETENCIES<br>● Acting Strategically: Identifies key issues and relationships relevant to achieving goals, and commits to a course of action to accomplish goals based on known and unknown variables. Finishes tasks promptly and critically reviews work for accuracy and quality. Consider the impact of work on others.<br>● Prioritizing and Organizing Work: Allocates time and attention based on what is most important to achieve key goals and objectives. Effectively organizes and balances tasks and priorities to keep multiple projects on track.<br><br>QUALIFICATIONS/EXPERIENCE/JOB KNOWLEDGE<br>● Preferred experience in serving individuals with disabilities or four (4) years full-time equivalent experience in a supervisory position, preferably in the field of mental health or a related field.<br>● Preferred four (4) year degree in human services, social work, or related field.<br>● Experience in supervising staff and following licensing and regulatory guidelines.<br>● Needs a clear understanding of administrative procedures and personnel management.<br>● Needs an understanding of Medicaid guidelines as they relate to programming.<br>● Working knowledge of Google Workspace and the ability to learn other software programs as required for this position.<br><br>PHYSICAL REQUIREMENTS<br>● Constantly moves about to coordinate work.<br>● Regularly works in a fast-paced environment with multiple task deadlines.<br>● Regularly moves and positions objects weighing up to 50 pounds.<br>● Occasionally exposed to viruses and infectious conditions.<br>● Constantly alert and observant during working hours.<br><br>Reasonable accommodations may be made to enable individuals with disabilities or medical conditions to perform the essential functions.<br><br>SUPERVISORY RESPONSIBILITIES<br>● Will supervise assigned staff/families.<br><br>SPECIAL REQUIREMENTS<br>● Must have understanding, patience, and tact in dealing with individuals served and their families.<br>● Must be creative in problem-solving and planning.<br>● Must have the ability to maintain good working relationships with staff, professionals, and persons from other agencies.<br><br>SKILLS AND ABILITIES<br>● Working knowledge of Google Workspace.<br>● Aptitude to learn other software programs as required for this position<br><br>EMPLOYMENT VARIABLES<br>● Must have a good driving record that meets the minimum requirements for Evergreen and reliable transportation.<br>● Must pass drug screen and criminal background check.<br>● May be asked to work some evenings when required.<br>● Extensive travel in your company or personal vehicle (mileage role will apply).<br><br>WORKING ENVIRONMENT<br>● May be required to work in a variety of settings and environments, both indoors and outdoors.</div>
