Salary Range: $29.00 - $33.35 Hourly

POSITION SUMMARY: The Facility Medication Nurse Coordinator is responsible for ensuring facility MAR and PACE MAR coincide. Is required to provide up to date medication orders as well as ensuring all medications are signed quarterly and provided to facility. Must provide signed orders to assisted living facility in a timely manner. Communicates regularly with medication nurses regularly. Covers providers when needed. This position may administer medications and treatments, provide participant, family, facility teaching, and supports those activities necessary to the preservation of organization in the assisted living environment. Participates in the communication of the participant’s condition to the medical providers in the PACE Clinic.
RESPONSIBILITIES AND DUTIES:
ESSENTIAL JOB FUNCTIONS:
- Provides medication reconciliation and communicates with 20+ assisted living facilities biweekly,
- Rounds regularly in each ALF and serves as a direct interface with these facilities to improve safety and satisfaction
- Manages Medication Administration Record (MAR) between PACE and assisted living facilities for 200+ participants
- Ensures accurate Implementation of orders written by PACE medical providers
- Communicates effectively to the assisted living facility staff, IDT and to family and caregivers, including providing instructions regarding medications and treatment.
- Builds positive relationships with staff and leadership in ALF settings
- Effective at functioning with limited direct supervision, able to multi-task and prioritize work tasks at a high level
- Keeps Medication Room Manager and assisted living Facility Liaison informed on facility issues as they arise
- Assists medical providers with medication reduction by providing detailed list of medication discrepancies
- Performs daily medication count and inventory tracking of clinic stock medications and narcotics
- Assists with  monthly inventory and record keeping of Emergency Cart
- Assists with daily tracking of medication refrigerator and laboratory freezer temperature logs
- Tracks, monitors and provides reports to medical providers of participants receiving antibiotic therapies
- Provides general nursing care to participants and documents test results and examinations in the EMR
- Observes, records, and reports participant’s condition and reaction to drugs and treatments to physicians
- Maintains the confidentiality of all company procedures, results, and information about participants, clients or families
- Handles potentially infectious specimens with appropriate biohazard precautions, and practices universal precaution
- Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certification
- Other duties as requested/assigned

Qualifications
- Graduate of an accredited Licensed Practical Nurse Program and licensed as a professional in the State of Colorado is required
- One year of health care experience in hospital, long-term or home health care setting with 1 year of experience with a frail or elderly population is highly desired
- Must be proficient with computer applications such as email, navigating the internet, using electronic timekeeping, and Electronic Health Records
- Knowledge of medical equipment and instruments
- Skill in applying and modifying the principles, methods and techniques of practical nursing to provide on-going care for participants
- Ability to perform phlebotomy functions
- Skilled in developing and maintaining department quality assurance
