Job Description

We are looking for an experienced, reliable and compassionate Support Worker to support a 30-year-old male participant living with Duchenne Muscular Dystrophy (DMD).

The participant is wheelchair-dependent and requires support with personal care, wheelchair transfers, domestic tasks and daily living activities.

This is an opportunity for a Support Worker who is confident working with complex physical support needs and understands the importance of safe, respectful and person-centred care.

Consistency, reliability and the ability to develop a positive working relationship with the participant are highly valued.

About the Participant & Support Role

The participant lives with Duchenne Muscular Dystrophy, a condition that results in significant physical support needs.

Support will focus on helping the participant maintain their:

  • Health and wellbeing

  • Participation in daily routines

The role may involve hands-on personal care, wheelchair transfers, domestic support and other daily living assistance in accordance with the participant's individual support plan and established support procedures.

We are particularly interested in Support Workers who are comfortable and experienced in supporting people with significant physical disabilities and mobility limitations.

Available Shifts

There are several ongoing opportunities available.

Weekly

  • Wednesday: 8:00 AM – 10:00 AM
  • Sunday: 8:30 AM – 10:30 AM
  • Sunday: 9:00 AM – 11:00 AM
  • Sunday: 8:00 PM – 10:00 PM

Fortnightly

  • Wednesday: 8:00 PM – 10:00 PM
  • Thursday: 8:00 PM – 10:00 PM
  • Saturday: 8:00 PM – 10:00 PM

Support Workers may express interest in one or multiple shifts depending on their availability.

Consistency across agreed shifts is highly valued, as regular support can help establish familiarity, trust and continuity for the participant.

What We're Looking For

We are seeking a Support Worker who is:

  • Experienced in disability support and complex physical care
  • Comfortable supporting a wheelchair-dependent participant
  • Experienced or confident with wheelchair transfers
  • Comfortable providing personal care
  • Reliable and punctual
  • Patient, respectful and compassionate
  • Physically capable of safely performing required support tasks within their training and role
  • Comfortable following manual-handling and transfer procedures
  • Attentive to participant safety and wellbeing
  • Respectful of dignity, privacy and personal boundaries
  • Able to follow individual support plans and established procedures
  • Confident communicating with the participant and support team
  • Committed to promoting independence rather than doing everything for the participant
  • Able to maintain appropriate professional boundaries
  • Consistent with agreed availability

Experience supporting people with Duchenne Muscular Dystrophy, muscular dystrophy, progressive neuromuscular conditions, spinal cord injuries, cerebral palsy or other significant physical disabilities would be highly regarded.

Key Responsibilities

Depending on the participant's individual support plan and needs, responsibilities may include:

  • Providing person-centred disability support in the participant's home.
  • Assisting with personal care and activities of daily living.
  • Supporting safe wheelchair transfers in accordance with the participant's established procedures and any required equipment.
  • Assisting with mobility and positioning as required.
  • Supporting the participant to maintain comfort, dignity and independence.
  • Assisting with domestic tasks and household routines.
  • Supporting meal preparation and other daily living activities where required.
  • Following appropriate manual-handling and workplace safety procedures.
  • Supporting the participant to participate in their usual routines and activities.
  • Respecting the participant's choices, preferences, privacy and boundaries.
  • Monitoring and communicating relevant changes in the participant's support needs.
  • Maintaining accurate shift notes and required documentation.
  • Communicating professionally with the participant, family/support network and Chosen Family team.
  • Promptly communicating any concerns, incidents or changes affecting the participant's safety or support.
  • Providing reliable and consistent support across agreed shifts.

Essential Requirements

Applicants should have:

  • NDIS Worker Screening Check
  • Current First Aid Certificate
  • Relevant disability support or personal care experience
  • Experience or demonstrated competency in supporting people with significant physical support needs
  • Experience with or willingness to undertake relevant manual-handling and transfer procedures
  • Strong communication and interpersonal skills
  • Commitment to safe, respectful and person-centred support
  • Ability to maintain professional boundaries
  • Reliability and punctuality
  • Ability to follow participant-specific support plans and procedures
  • Availability for at least some of the advertised shifts

A valid Australian Driver's Licence and access to a reliable vehicle may be advantageous depending on the participant's location and support requirements.

Additional qualifications or training in disability, individual support, nursing, allied health, manual handling or personal care will be highly regarded.
