Hope and Change is a company in rapid growth and we need to add more wonderful experienced support workers across Brisbane’s southern areas, and we are looking for capable, reliable people who want more than simply another casual support role, there might even be part time availability in the future!

Many of the people we support have complex health, personal care and behavioural support needs. We are looking for Support Workers who combine compassion with capability — people who are dependable, communicate well, understand professional boundaries and recognise the responsibility that comes with supporting someone in their own home.

The role

Depending on the individual needs of the person you are supporting, your work may include:

  • personal care and daily living support
  • community access and social participation
  • manual handling and mobility assistance
  • medication assistance and accurate documentation
  • implementing individual support and behaviour support plans
  • catheter and continence support
  • PEG feeding support
  • mental health and psychosocial disability support
  • infection prevention and control
  • maintaining accurate progress notes and service records
  • working collaboratively with families, clinicians and other members of the support team.

We are particularly interested in Support Workers who already have experience supporting people with complex or higher-care needs.

About you

The people who do well at Hope and Change are reliable, calm, professional and consistent

You understand that excellent disability support is not simply about being caring. It also requires sound judgement, communication, accountability, accurate documentation and respect for the rights, choices and dignity of the person you are supporting.

You will ideally have:

  • at least 12 months’ experience working in disability, community or complex care
  • Certificate III or IV in Individual Support (Disability), or a comparable qualification
  • current First Aid and CPR certification, or willingness to obtain
  • a current Australian driver’s licence
  • access to a reliable and appropriately insured vehicle
  • NDIS Worker Screening clearance and other required screening clearances, or the ability to obtain them
  • confidence using mobile applications and completing electronic support documentation.

Experience or additional training in manual handling, medication assistance, PEG feeding, catheter care, continence support, mental health, positive behaviour support or trauma-informed practice will be highly regarded.
