Job Description

The Evaluating Clinician works with Medical Director and QHPs in follow up to initial evaluations and assessments; performs subsequent special consultations when indicated by assessment and initial medical evaluation following established protocols.

What You'll Do:

Essential Job Functions:

  • Ensure that Comprehensive Assessments to establish and/or maintain collaborative relationships with the health provider community, on behalf of the WeCARE Program

  • Perform medical evaluation and assessments which include:

1. Medical history/prior WeCARE history

2. Physical examination

3. Review of prior WeCARE documentation (when applicable)

4. Review clinical documentation provided by the individual (when available)

5. Medical, mental health, substance use, vocational, educational, social history, etc.

  • Perform Sequential Evaluation

  • Execute best practices, adopt information technology updates, incorporate predictive analytics into patient care flow redesign decisions; and respond to operations issues when they are prioritized with urgency

  • Create participant-specific report(s) listed below after conducting medical evaluations

  • Conduct the sequential evaluation process as defined by the SSA (Social Security Administration)

  • Outline diagnoses

  • Assist in determining the individual’s Functional Capacity Outcome (FCO)

  • Conduct Medical evaluation when the FCO cannot be determined on the basis of the Clinical Assessment or Clinical Re- assessment

  • Identify operational issues and facilitate corresponding process improvements in close collaboration with Medical Director

  • Ensure that medical services are delivered within HRA guidelines

You'll be a Great Fit For this Role If:

  • New York State licensed doctor (MD) with training in internal medicine, family practice, physical medicine and rehabilitation, or in other medical disciplines as designated by HRA who reviews the Clinical Assessments/Clinical Reassessments immediately following their completion when indicated

  • Experience managing large scale projects and driving change initiatives, including as-is process mapping and the design and rollout of new processes and standard operating procedures
