*Posted Pay Range is based on full time annualized.

Harbor Health operates two Elder Service Plan Programs of All-Inclusive Care for the Elderly (PACE), a program that allows frail elders to continue to live in their homes and communities. Our Elder Service Plan has two medical and day centers in Mattapan and Brockton, MA and cares for more than 550 participants. We are currently seeking a part time Chaplain to join our team.

Role:

Under the supervision of the ESP Social Services Manager, and working with the Interdisciplinary teams (IDT), the Chaplain provides spiritual leadership and counseling to individual and groups of participants as well as caregivers and family of the Elder Service Plan (PACE) program. The Chaplain is responsible for the organization, supervision and administration of the religious program as well as other religious activities that may be identified.

Responsibilities:

  • Provides complete services of care, worship and instruction, spiritual leadership and counseling and spiritual assessments.
  • Participates in IDT meetings and care planning as appropriate.
  • Maintains timely documentation of participant contact in the electronic medical record.
  • Provides Christian Education and spiritual support.
  • Supports participants through bi-weekly prayer and individual visits on a rotating schedule.
  • Provides support for both Boston and Brockton locations, including visits to residential facilities and individual homes in the catchment area, as needed.
  • Hosts sessions and teachings on the topics such as, but not limited to:
  • Contemplative Practices, such as meditation and prayer
  • Creating a Happiness Maintenance
  • Grief and Loss
  • Forgiveness
  • Maintaining healthy relationships
  • Responds to participants questions related to mortality, spiritual peace and “non-traditional” approaches to faith.
  • Works in collaboration with the IDT and Social Services Manager to provide comprehensive spiritual care.
  • Attends Ethics meetings and Staff Enrichment trainings to add Theological perspective, as requested.
  • Works with clergy of other faiths to respond to the needs of the participants, including administering sacraments and last rites.
  • Participates in individual and group supervision as requested by manager. Participates in Social Services department meetings and trainings as requested by manager.

Requirements:

  • Undergraduate Degree and a Master of Divinity degree consisting of a three-year education from an accredited seminary or school of theology
  • At least two years of pastoral/spiritual leadership experience in a specialized non-congregation or non-profit setting
  • Contextual education training may satisfy at least one year of experience requirement.
  • Computer literate
  • Proven Organizational skills
  • Proven oral and written communications skills.
  • Experience working with a Geriatric population preferred.
  • Must be able to travel timely between Harbor ESP sites and occasional home visits with participants.

The salary range and/or hourly rate listed is a good faith determination of base compensation that may be offered to a successful applicant for this position at the time of this job advertisement.

When determining actual base salary and/or rate, several factors may be considered as applicable (e.g. location, years of relevant experience, education, training, and other factors as permissible by law). As such, most offers will not be at the top of the salary range.

Hours can be flexible, with a minimum of 3 days in the office. Center hours are 8:00 AM - 4:30 PM.
