About Us

360 Health Management is a regional disability support provider servicing Bendigo, Shepparton, Swan Hill and Seymour, offering a range of services to our NDIS participants from Support Coordination, Behaviour Support, Respite and much more.

We are looking for people who are eager to take their next step within the industry or those who are new in the world of Support Coordination with no previous experience but want to help people! Responsibilities:

  • Assist clients in navigating social care services and resources.
  • Provide administrative support to the social care team, including scheduling appointments and maintaining records.
  • Conduct initial assessments to identify client needs and develop appropriate support plans.
  • Collaborate with external agencies and service providers to ensure comprehensive care for clients.
  • Maintain accurate documentation and case notes in compliance with organisational policies.
  • Participate in team meetings and training sessions to enhance service delivery and personal development.

About you:

  • Agile - no two days are ever the same in this role
  • Work well under pressure, as you are working with someone's life, things will often move and change, requiring ability to navigate this to support clients
  • Strong attention to detail and love for numbers as you are working with their individual funding budget and plan
  • Outstanding organisation and time management skills, to ensure that all participants are being supported equally

Requirements:

  • A minimum of a high school diploma or equivalent qualification level
  • Previous experience in a customer service or support role is advantageous but not essential
  • Strong communication and interpersonal skills, with the ability to build rapport with clients
  • Basic understanding of social care principles and practices
  • Proficient in using computer software and applications for documentation and communication
  • A compassionate and empathetic approach to working with vulnerable individuals
  • Drivers licence and access to a reliable car

360 Health Management established its NDIS presence locally in Shepparton in early 2020. Belinda (Managing Director) could see a local support gap that needed to be filled and worked hard to find a positive and reliable team to assist our community.

Moving forward 5 years, 360 Health Management now have over 60 staff members that have an abundance of skills, knowledge, and compassion to support you, your family and the community.

We now offer a range of NDIS services and supports with flexibility to tailor your supports to your individual needs.
