Minimum Qualifications
The PACE Life Program Enrollment Coordinator assists the Enrollment Specialist with screening and intake assessments. This position also follows up on visits and Medicaid applications for eligible applicants to the program. This position also coordinates center assessments and works closely with the County Welfare Office and Area Agency on Aging on an ongoing basis for enrollment into the program.
The PACE Life Program Enrollment Coordinator also assists in marketing and sales efforts including participation in health fairs, presentations to the community and one -- on one follow up with interested parties.
This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to team work and cooperation.
QUALIFICATIONS
EDUCATION: High School Diploma required; Bachelor's degree preferred.
LICENSE/CERTIFICATION: Valid NJ Driver's License, Registration and Insurance are required.
EXPERIENCE: Marketing or sale experience in a health care, insurance or similar environment preferred. Computer proficiency in Microsoft Word Office Applications required. Bi-Lingual preferred. Minimum 1 year working in senior healthcare preferred.
PERFORMANCE EXPECTATIONS
Demonstrates the competencies as established on the Assessment and Evaluation Tool for this position.
WORK ENVIRONMENT
Employee must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social service disciplines who work together to manage the PACE participants' care. The work setting is in an Adult Day Health Center and primary care clinic environment with moderate noise levels and controlled temperatures. Requires manual and finer dexterity and eye-hand coordination; the ability to use department equipment; and the ability lift/carry up to 30 pounds using appropriate body mechanics with reasonable participants, customers and vendors and staff.

This position reports to department leadership.
