About the Role
Chosen Family is seeking compassionate and participant-focused Enrolled Nurses (ENs) to provide high-quality nursing and disability supports across home, community, respite, and Supported Independent Living (SIL) environments.
The EN will work collaboratively with participants, families, Registered Nurses, allied health professionals, and support teams to deliver safe, person-centred, and trauma-informed care that promotes participant wellbeing, independence, dignity, and quality of life.

We have a strong focus on supporting individuals with complex needs, including those experiencing mental health challenges, trauma, or social isolation. Our team takes a respectful and understanding approach, ensuring support is always person centred and identity affirming.
We work closely with support coordinators, plan managers, allied health professionals, and community organisations to build strong referral pathways and collaborative outcomes.
