Job Shift: M-F Day ShiftChartiers Center is a private, non-profit corporation funded by state, county, third party  insurances and private funds. Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day, and recreational services for adults with intellectual disabilities, homeless outreach, and housing program. The Janus Program is a new and exciting model of care that uniquely assists participants to living on their own and learn desired skills. The Enhanced Case Manager is a member of the multi-disciplinary team that works closely with program participants. Although we embrace a "whatever it takes" approach the Enhanced case manager provides personalized and specialized Service Coordination to participants of the Janus Program.
As an Enhanced Case Manager with Chartiers Center the core job duties include:
- Coordinate and oversee the delivery of benefits, treatment and support services to participants.
- Independently assess participant needs and develop individualized plans that provide the intensity and frequency necessary.
- Meet with participants on a frequent basis to provide needed interventions and monitor progress.
- Participate in rotating on-call services to support the Janus program participants.

Minimum

Qualifications:
- Bachelor’s degree in human services and 3 years of mental health experience OR
- Master’s degree in human services with 2 years of experience
- Valid Driver’s License Required
