Position Summary:
This role will provide comprehensive outpatient care for patients with endocrine and metabolic disorders and will function as a collaborative provider, managing a broad spectrum of chronic endocrine conditions, delivering patient-centered care, and supporting long-term disease management.
This role requires eligibility for a 103 License (Nurse Practitioner furnishing number) and a strong clinical background, preferably endocrinology, metabolic health, and hormone therapy.
Duties and Responsibilities:
- Diagnose, treat, and manage a wide range of endocrine and metabolic conditions, including Diabetes types I and II, Obesity & Weight Management, PCOS, Thyroid disorders, Adrenal and Pituitary disorders, Metabolic syndromes (PMOS, MASLD).
- Provide and manage evidence-based hormone therapies, including Gender-affirming hormone care and Hormone replacement therapy (HRT) for post-menopausal women. Monitors treatment effectiveness and adjusts therapy plans as appropriate.
- Perform comprehensive patient evaluations, including history, physical exams, and interpretation of diagnostic tests
- Develop individualized treatment plans focused on lifestyle, pharmacologic, and technological interventions
- Prescribe medications, durable medical equipment, and diabetes technologies (e.g., CGMs, insulin pumps)
- Educate patients on disease management, medication adherence, and lifestyle modification
- Collaborate with physicians, dietitians, diabetes educators, behavioral health providers, and other specialists
- Coordinate referrals and manage continuity of care
- Participate in interdisciplinary case discussions when appropriate
- Proactively assists the patient and family through all facets of accessing care at the Clinic and elsewhere, serving as a resource to resolve patient and family concerns.
Position Requirements:
- Master’s degree in nursing (NP program)
- Active and unrestricted Nurse Practitioner license in California
- Eligibility for or possession of a 103 License (NP furnishing number)
- National board certification as an NP (e.g., FNP, AGNP)
- Current DEA license and BLS certificate
- Minimum of 1–2 years of clinical experience preferred (endocrinology experience strongly preferred)
- Basic knowledge of local, state and federal regulations relating to care of patients in a clinic setting; and prior experience with county, state and federal programs such as CHDP, FPACT, EWC, Medi-Cal and Title X is preferred.
