
To actively schedule and navigate employees served by Hendricks Regional Health (HRH) Employer Wellness Center partnerships over the phone and in-person. To effectively provide scheduling services, support, education for employees served by HRH employer wellness centers. Scheduling and education support may include: primary care physicians, specialists, marketing, physical therapy, transitions of care professionals, revenue cycle specialists and community resource contacts, while functioning with professional autonomy under the supervision of the Outreach Coordinator/Daily Operations Manager of Wellness and Population Health.
Essential Responsibilities:
1. Primary scheduler for the organization’s overall employer wellness centers: efforts including, but not limited to, the following
a. Support HRH Employer Partners – Manage patient navigation needs from our corporate partners. Working with appropriate staff within HRH and employer partners to ensure proper network coverage and information is disseminated. Work with Tiered Network coordinator as directed for additional support. Serve as main phone support and electronic informational support to all employer partners needing scheduling services.
b. Work with our clinic providers and wellness clinical team members to guide patients to properly covered services.
c. Assist patients as contacted on various referral needs under the guidance of our Tiered Network Navigator.
d. External events – Partner with wellness team members and marketing for participation in community-wide health events and screenings. Responsibilities include activity set-up, participant follow-up, planning and procurement of needed materials and supplies.
e. Communications support – Utilize a variety of social media and other communication platforms to engage the community, HRH associates and wellness program participants. Work with wellness and other health experts to provide appropriate, timely wellness information to the appropriate audience for increased relationship-building opportunities. Example: a regular information update to parents of students in Avon Community School Corporation, one of our key corporate partners.

Preferred Education and Experience:
1. Excellent knowledge of Word, Excel- Office Suite.
2. Knowledge of eCW a plus.
3.Familiarity with HRH physician practices a plus.
