Long Term Care Eligibility Specialist
Jefferson County invites you to join our Community Assistance Division as aLong-TermCare (LTC)EligibilitySpecialist.AsaLTC Eligibility Specialist, youwillplay a critical role in ensuring older adults and individuals with disabilitieshavetimelyaccess to essential publicassistancebenefits. Youare responsible foraccuratelydeterminingeligibility for Long Term Care Medicaid, Food and Cash Assistance programs while providing exceptional customer service to some of the county's most vulnerable residents.
LTC Eligibility Specialists manage an ongoing caseload andare responsible formaintainingregular communication with customers, family members, medical providers, nursing facilities, case management agencies, and community partners throughout the eligibility process. This position requires balancing competing priorities, meeting strict timeliness standards, and navigating complex policies and regulations.
Responsibilities include gathering, verifying, and documenting financial and non-financial information, researching complex eligibility factors, and makingaccurateeligibility determinationsin accordance withfederal and state regulations.
EMAS Eligibility Specialist

SCHEDULE:
This position is available for schedules offive-days a week (8-hour workdays Monday-Friday) or four-days a week (10-hour workdays) This position is hybrid, with remote work and in-office workisdeterminedby team and agency needs. Note that scheduling arrangements may be adjustedin accordance withoperational and business needs.
COMPENSATION:
AnticipatedHiringSalary$25.30 USD Hourly
Compensation will bedeterminedbased on education,experienceandskills.

ESSENTIAL DUTIES:
Provide excellent customer service provided to internal and external customers and community members.
Utilize critical thinking and meticulousstrongattention to detail to ensureaccuratedata entry andeligibility determinations.
Conduct effectiveinterviews with customers requesting publicassistanceandcommunicateclearly in person, in writing, and over the phone with customers, coworkers, and community partners.
Identifyand assess customer needs to provideappropriate solutions, guidance, and resources.
Investigatecustomer concerns and complaints toidentifyroot causes and implementappropriate resolutions Adapt to the changing business needs and operation priorities of the agency.
Educate customers about available community resources and support services, enhancing their ability to access essential programs.
Identify and report potential fraud cases to the Investigations Unit.
Maintain organized andaccuratecase documentation
Demonstrate self-motivation and the ability to work independently while contributing positive to team collaboration.
Troubleshoot system and case issues as needed to ensureaccuratecase processing.
Ensure compliance with state and federal timeliness expectations for processing customer cases anddeterminingeligibility for publicassistanceprograms.
Maintain strict confidentiality and protect customer data and privacy.
Effective organizational skills.
Participate in required training and successfully complete assessments tomaintainand expand program knowledge and skills.
Performs other duties as assigned.

Minimum Qualifications:
Experience:One -yearrelated experience
Education:High School diploma or GED
*Note: An equivalent combination of education and experience is acceptable.
Preferred Qualifications:
Experience in determining eligibility in Medicaid and SNAP benefits utilizing the Colorado Benefits Management System (CBMS).
Experience in customer service and de-escalation techniques
Data entry experience in complex computer databases.
Experience in a social/human services field.
Spanish speaking skills preferred, but not required

Additional Job Information:
