Job Description

JOB SUMMARY:

Provides patient/resident care activities under the direction of Registered Nurse and/or Licensed Practical Nurse. Assists patient/residents with activities of daily living, provides for personal care, comfort and assists in the maintenance of a safe and clean environment for patient/residents.

JOB DUTIES:

  • Observation Skills: Recognizes and understands the resident/patient needs and change of condition. Reports any changes to attending nurse.
  • Implementation of Plan of Care: Works under the direction of nursing staff to implement the appropriate interventions within their scope of practice. Uses appropriate technique in transfer and transporting patients.
  • Collaboration: Demonstrates the ability to anticipate the needs of other health team members and impacts relationships in a positive manner. Supports the goals of each member of the team in the provision of the plan of care. Participates in the orientation of new staff.
  • Responsiveness: Demonstrates ability to respond to patient and family needs, answering call lights/alarms, in a timely manner.
  • Documentation: Documents vital signs, intake and output, physical care, and ADLs appropriately via computer/flow sheet in a timely manner. Use of the Admission and Assessment Record. Uses military time.
  • (For spec. depts.) Performs accurate and timely order entry, prioritizing well, and communicates appropriately with providers regarding orders.
  • Cleanliness of Environment: Ensures equipment is properly taken care of after patient use. Performs tasks to maintain a clean and healthy patient care environment in a timely manner. As assigned, maintains cleanliness of staff/supply areas.
  • Performance Improvement: Participates in processes to improve patient care outcomes and satisfaction

Minimum knowledge, skills, and abilities:

  • High school graduate or GED preferred.
  • Completion of the Certified Nursing Assistant program OR be listed as having passed CNA competency testing on the Iowa Direct Care Worker registry. May be certified as a medic or Combat Lifesaver (through military duty) or have EMS certification as a base provider.
  • Current American Heart Association (AHA) Heart Code BLS required. New employees certify within three (3) months after hire date.
  • Ability to read and communicate effectively in English.
  • High degree of empathetic listening, tact and kindness toward others.
  • Effective verbal and nonverbal communication skills required.
  • Must work effectively as a member of a team.
  • Previous acute or long-term care experience preferred.
  • Technical and critical thinking skills required to perform duties.
  • Basic computer knowledge.
