
The Emergency Department Diagnostics Follow up Coordinator (DFC) RN, is responsible for the coordination for direct patient follow up or follow up through providers, related to aberrant diagnostic results on tests generated during Emergency Department or Walk-In-Urgent-Care Visits. The Diagnostic Follow up Coordinator collaborates closely with a designated CHEMA physician and the Nursing Director with ongoing development, implementation and evaluation of the diagnostic follow up program. They also manage and facilitate communication with patient and doctors in the community, to offer early interventions that promote patient safety, physician and patient satisfaction, and decrease possibility of liability associated with omissions.
Graduate from an accredited Associates degree program, BSN preferred.
Licensure required: Registered Nurse with current license to practice in NH
Certification required: None
Minimum of three years Emergency Department experience necessary to address a wide variety of patient concerns. Knowledge and skills pertaining to patient education and adult learners. Must have a reassuring phone demeanor and exceptional customer service skills, including service recovery. Microsoft Word and Excel skills required.
