Available Positions: Currently hiring Service Coordinators for NYC AND Long Island cases!

** previous Early Intervention Service Coordination experience required **

Service Coordinators are a vital part of the Early Intervention Program. Caseloads are available throughout the five boroughs of NYC and Long Island. This position has a unit payment based on 6-15 min.

What you’ll be doing at ACP Early Intervention:

The Service Coordinator (SC) helps families of children with a developmental delay navigate all aspects of the Early Intervention (EI) system by coordinating all services available to children across agencies, providing information on available services, resources and options. The SC provides case management to families by planning, coordinating, participating, and monitoring of the Individualized Family Service Plan (IFSP).

  • Communicate and assess families concerns, priorities and resources, strengths and needs.
  • Ensure services are provided as per the Individual Family Service Plan (IFSP)
  • Assist families to access community resources and services
  • Complete notes after each contact made.
  • Submit paperwork on a timely basis
  • Communicate with child’s EI team including providers, outside agencies, municipality and ACP-EI staff.

Knowledge, Skills, Abilities:

  • Knowledge of child development a plus
  • Effective written and verbal communication skills
  • Well Organized and effective time management skills
  • Must follow agency and municipality policy and procedures
  • Submit all documentation on a timely basis
  • Ability to work independently and be an effective team member
  • Available for daytime meetings
  • Spanish Speaker

Minimum Qualifications:

  • Degree in a social services field and/or experience with young children with developmental delays

Rates:

NYC- $11-$13 per unit based on experience

Long Island - $10-$12 per unit based on experience
