Salary or Pay Band: Pay Band 8, $42.04 to $52.44 (6 step range)

GENERAL ACCOUNTABILITY:
Nurse Navigator is responsible for the coordination, development, implementation, administration, evaluation, and delivery of patient navigation. The navigator will work collaboratively with patients to ensure individualized patient centred care is achieved in the timeliest manner possible. In collaboration with health system partners and clinicians, the Navigator acts as a clinical resource and liaison providing support, leadership, direction, and education to facilitate the coordination and continuity of services in matters related to the patient. This position reports to the Provincial Manager, Systemic Therapy and Outpatient Services.

QUALIFICATIONS:

Education

• Bachelor’s degree or Diploma in Nursing
• Completion of the Oncology nursing certification CON(C) or equivalent. Where this is not complete the successful candidate is required to obtain certification within two years of date of hire to the position.

Experience

• Five (5) years nursing experience.
• Demonstrated experience in leadership and/or program development and evaluation and/or patient education is required.
• Minimum 3 years of Disease Site and/or Navigator and/or Educator and/or Oncology nursing experience is required.
• For certain navigator positions, experience living or working in a specific geographic area and/or proficiency in a particular dialect or language may be considered an asset.
• If there are no fully qualified applicants, those with a minimum of 6 months
  working experience in the SCA Referral Centre within the past 2 years will be considered
  

Membership

• Licensed with the College of Registered Nurses of Saskatchewan (CRNS).

Knowledge and Skills

• Demonstrated ability to work collaboratively with multiple diverse stakeholders at all levels of responsibility both within the SCA and externally.
• Demonstrated ability to provide person-centered, trauma informed, culturally safe
  nursing support.
• Advanced patient assessment skills, virtually and in person, with the ability to complete initial and ongoing patient assessments (clinical and diagnostic reasoning) and provide high quality patient support through appropriate/prescribed technical, therapeutic and safety interventions.
• Ability to interpret detailed information and communicate effectively both orally and in writing.
• Requires high degree of judgment and problem-solving ability and critical thinking skills to identify, analyze and define problems and determine the most appropriate pathway.
• Ability to integrate relevant evidence-based best practice as standard performance.
• Ability to teach and present information effectively to persons over whom the incumbent may have no direct authority.
• Requires high-level skills in human relations to develop formal and trusted relationships with patients to navigate them effectively within the provincial health system and provide in-depth counselling and support.
• Demonstrated leadership skills required to lead Agency projects/committees.
  Ability to manage, prioritize and troubleshoot multiple tasks in various stages of
  completion.
• Competent in the use of computers and Microsoft applications.
• Maintains confidentiality.
