Discharge Planner / Social Services Director

Medical Hill Healthcare Center is seeking a compassionate, organized, and dedicated Discharge Planner / Social Services Director to join our team. This position plays an important role in supporting residents and families while coordinating care, resources, and successful transitions to the next level of care.

Pay Rate: $36.00–$37.00 per hour
Schedule: Full-Time

The Discharge Planner / Social Services Director is responsible for providing social services and discharge planning support to residents and their families. This position works closely with residents, families, healthcare professionals, and community resources to identify individual needs, develop appropriate plans, and promote the highest practicable level of independence and quality of life.

The ideal candidate is compassionate, organized, proactive, and able to communicate effectively while maintaining confidentiality and professionalism.

Essential Duties & Responsibilities

  • Develop individualized discharge and care plans by identifying resident needs, exploring available options, and establishing appropriate goals.

  • Assist residents in achieving the highest practicable level of self-care, independence, and overall well-being.

  • Provide medically related social services addressing residents’ physical, mental, and psychosocial needs.

  • Assess social, family, psychological, and emotional factors that may affect a resident’s care and discharge planning.

  • Provide emotional support, counseling, and guidance to residents and families coping with illness, disability, loss, grief, and significant life transitions.

  • Assist residents and families with healthcare-related decisions and care planning.

  • Coordinate discharge planning and help connect residents with appropriate healthcare providers, community agencies, and support services.

  • Communicate effectively with interdisciplinary team members to promote continuity of care.

  • Advocate for residents’ needs, preferences, rights, and overall quality of life.

  • Maintain accurate, timely, and confidential documentation in accordance with facility policies and applicable regulations.

  • Develop positive working relationships with residents, families, staff, healthcare providers, and community resources.

  • Assist residents and families in navigating available healthcare and social service resources.

Qualifications

  • Bachelor’s Degree required.

  • New graduates are welcome and encouraged to apply.

  • Strong organizational, communication, and interpersonal skills.

  • Compassionate and resident-focused approach.

  • Ability to maintain confidentiality and handle sensitive information professionally.

  • Strong problem-solving and critical-thinking skills.

  • Ability to work collaboratively with residents, families, healthcare professionals, and community agencies.

  • Knowledge of discharge planning, social services, healthcare resources, or case management is preferred but not required.
